About the Role

Knox Public Health is hiring a Registered Nurse (RN)-Complex Medical Help (CMH) Focus to join our Public Health Education and Nursing (PHEN) division. This position serves as the lead nurse for the CMH program and other community health initiatives. The RN will be primarily responsible for coordinating care for clients with special healthcare needs and providing education, outreach, and direct services to vulnerable populations.

This is a community-based nursing role that offers variety, autonomy, and the opportunity to make a real difference in the lives of families across Knox County.


Key Responsibilities

Primary Nurse – CMH Program

  • Coordinate all aspects of CMH program services.
  • Conduct home visits for children with special healthcare needs.
  • Perform comprehensive client assessments and periodic re-evaluations.
  • Develop, implement, and revise individualized care plans.
  • Educate clients and caregivers on disease prevention and care strategies.
  • Obtain physician orders and ensure documentation meets standards.
  • Facilitate referrals and enrollment into programs such as Women, Infants, and Children (WIC) and CMH.
  • Coordinate interdisciplinary goals with physicians, therapists, and aides.
  • Ensure services are billable under CMH public health nurse consultative service.
  • Make referrals to Help Me Grow, when appropriate.

Lead Poisoning Case Management

  • Provide education and resources to families of children with elevated blood lead levels.
  • Ensure follow-up testing is completed by medical providers.

Community Resource Coordination

  • Maintain knowledge of local resources and services.
  • Make and document referrals to community agencies and programs.

Immunization Services

  • Serve as backup coordinator for Danville Sanctuary monthly immunization clinics.
  • Administer and document vaccines for children and adults during outreach and on-site clinics.
  • Reconcile and update data in the statewide immunization registry as required.

Newborn Home Visits

  • Act as the primary nurse conducting newborn home visits.
  • Document visits in Electronic Medical Records (EMR) and follow up on any concerns or referrals.

Infant Safe Sleep Program

  • Serve as the lead nurse for the Safe Sleep initiative.
  • Conduct home visits and ensure compliance with documentation standards.

Communicable Disease Control

  • Provide backup support for communicable disease case investigations.

Qualifications

  • Bachelor’s degree in Nursing (BSN) – required
  • Licensed RN in Ohio – required
  • Minimum 1 year of RN experience – required
  • Active BLS (Basic Life Support) certification – required
  • Proficient in EMR systems
  • Excellent communication, customer service, and organizational skills
  • Proficient in Microsoft Office Suite (Word, Excel, PowerPoint)
  • Valid Ohio Driver’s License and personal vehicle insurance
  • Strong multitasking skills and ability to manage priorities

Preferred Experience

  • Community or public health nursing
  • Knowledge of CMH, WIC, and Help Me Grow programs
  • Pediatric or home visit nursing background
